Where is the area for the instructor stamp?

Uh, on page 2 where it says "The instructor’s stamp goes in this box" it is a big box!

If you do not see that, refresh your browser page as you may be looking at an old cached copy.

Clicked on your link and it still looked old. Did a refresh and there is was, the new application.

Now how long has it been out there? I have been checking at least once a day and apparently been looking at an old cached version on my PC.
